How does token billing actually work?
You purchase tokens in advance — think of them as prepaid clinical credits. Tokens are deducted when an appointment is paid, which happens when a Patient Health Check is sent to the patient. The number of tokens deducted depends on your specialty and appointment type (e.g. an ENT consult costs 25 tokens, an ENT follow-up costs 20). All features — AI overview, transcription, document generation, SMS, and letters — are included in that single token spend. No add-ons, no surprises.
Do tokens expire?
No. Tokens never expire. You can buy tokens now and use them three months later. This is intentional — clinicians work seasonally, take leave, and have variable caseloads. There is no pressure to use tokens by a certain date, and your balance simply waits for you whenever you're ready to see patients.
What happens to my account when I'm on leave or doing locum work?
Nothing at all. There is no subscription to pause or cancel, and no monthly fee running in the background. Your token balance, patient records, templates, and settings are preserved exactly as you left them. When you return — whether that's next week or in six months — your account is ready immediately. There is no re-activation process and no charge for the time away.
What's included in a paid appointment?
Every paid appointment unlocks the complete ApolloScribe feature set for that patient encounter: pre-consult health check survey delivery, AI Patient Overview synthesis, Insights & Actions risk flags, medical-grade voice transcription (Deepgram), one-click generation of all documents (Patient Letter, Consult, Billing, Clinical Snapshot), automated patient email, and all SMS notifications. There are no separate charges for AI model usage, transcription minutes, or outbound SMS.
How do volume tiers work? Do I need to buy a certain amount to get discounts?
Volume tiers work like tax brackets — each rate applies only to the tokens within that band, not to your entire purchase. For example, if you buy 1,000 tokens for a specialist practice: the first 200 cost $0.10 each, the next 600 cost $0.09 each, and the remaining 200 cost $0.07 each. You never need to commit to a large purchase to access discounts — you automatically benefit from whichever tier each portion of your purchase falls into. There are no lock-in commitments to earn a better rate.
Can I set up automatic token top-up?
Yes. In your billing settings you can configure a threshold (e.g. when your balance drops to 50 tokens) and a top-up quantity. When the threshold is hit, Stripe processes the purchase automatically and you receive an invoice by email. Volume tier pricing applies to auto top-ups the same way it does to manual purchases. We also recommend enabling auto top-up to avoid any service interruption during a busy consulting session.
Are there any other charges — per user, per month, or setup fees?
No. There are no per-user fees, no monthly platform fees, no setup fees, and no cancellation fees. The only cost is the tokens you purchase, spent only on appointments you actually see. Demo patients are always free — you can test workflows with no token cost. All new practices also receive 1,000 free tokens on signup with no credit card required.
